This is a fully remote position, open to applicants in North Carolina.
• Gather, organize, document, and convey data for primary health assessments and comprehensive medical histories.
• Create time-sensitive treatment plans for specific case-managed patients or populations.
• Conduct patient assessments via telephone adhering to established protocols.
• Prioritize patient care and monitor as well as evaluate progress towards defined goals.
• Deliver coordinated, efficient, and effective healthcare to beneficiaries.
• Supervise discharge-planning initiatives.
• Aid healthcare finders in identifying patient referral requirements.
• Negotiate, draft, finalize, and manage product/service agreements.
• Implement utilization review criteria and report quality or risk management concerns.
• Utilize VA/DOD Clinical Practice Guidelines to standardize care delivery.
• Assess healthcare financial aspects and population health resource considerations.
• Employ research tools to analyze practices, opportunities, and outcomes.
• Manage information systems and technology for communication regarding individual or population health and outcomes.
• Gather, track, trend, and analyze data utilizing computers and software applications.
• Design and deliver computer-generated outcome analyses, educational programs, and project presentations.
• Leverage pharmaceutical knowledge to assess effects, side effects, and complications.
• Adhere to KBR safety, quality assurance, attendance, training, and other organizational requirements.
• Undertake additional assignments and duties as necessary.
• U.S. Citizenship is mandatory.
• Secret Clearance or Top Secret Clearance, or the capability to obtain a clearance.
• Possess a current, full, active, and unrestricted license as a Registered Nurse or Licensed Clinical Social Worker in any U.S. State, the District of Columbia, Guam, Puerto Rico, or the U.S. Virgin Islands.
• At least two (2) years of nursing or case management experience is required.
• Certification as a case manager from a recognized certifying organization is necessary, or the candidate must obtain certification within the first year of employment.
• Proven ability to comprehend diagnoses.
• Familiarity with patient education principles and management of complex medical, psychosocial, and financial issues.
• Capability to seek feedback from peers, professional colleagues, clients, and outcomes research.
• Proficient in consulting with healthcare providers and negotiating with external providers.
• May be required to acquire and maintain clinical practice privileges.
• Ensure compliance with credentialing requirements in good standing at a local MTF.
• Excellent oral and written communication skills in English.
• Strong skills in team coordination, facilitation, and communication.
• Knowledge of local, state, and federal laws and regulations, professional codes of ethics, and practice standards.
• Must hold a current Basic Cardiac Life Support (BCLS) certification from the American Heart Association.
• Compliance with safety, health, environmental, quality assurance, attendance, training, and other organizational requirements is essential.
• Previous military experience is preferred.
