Social Worker – Palliative Care

This is a fully remote position, open to applicants in Pennsylvania.

📋 Description

• Conducts and records psychosocial assessments along with care coordination.

• Offers crisis intervention to improve coping strategies for patients and families.

• Participates in the discharge process to determine post-acute resources.

• Delivers education on psychosocial aspects related to health conditions.

⛳️ Requirements

• A Master's Degree in Social Work is mandatory.

• Minimum of 5 years of post-graduate experience as a Social Worker in a healthcare environment.

• Must possess a Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) credential.
