Social Worker

This is a fully remote position, open to applicants in United States.

📋 Description

• Conducts thorough psychosocial evaluations to identify the strengths, barriers, and support needs of patients and their families via video technology and telephone communications.

• Offers grief and bereavement support to patients and families, including anticipatory grief counseling and assistance with funeral services.

• Screens for and addresses mental health issues such as depression, anxiety, and caregiver stress.

• Evaluates safety risks, including suicidal ideation, abuse, neglect, and hazardous living conditions.

• Participates in weekly interdisciplinary team (IDT) meetings to review and discuss patient care plans.

• Provides medical social work case management for a designated group of patients and their families.

• Evaluates and responds to the psychosocial, practical, financial, and legal concerns of patients and their families/support systems.

• Aids patients and families in accessing and utilizing community resources.

• Guides patients and families through the navigation of social and healthcare systems.

• Mediates disputes between families and care networks.

• Completes financial needs assessments and assists in navigating Medicare, Medi-Cal, Disability, Social Security, and Veterans Affairs benefits.

• Assesses spiritual and medical needs, communicating these promptly to the interdisciplinary team.

• Establishes and nurtures relationships with community resources.

• Identifies issues, objectives, interventions, and frequency of visits to complete the social work Plan of Care.

• Facilitates advance care planning, including the completion of advance directives and POLST forms.

• Ensures documentation is completed within 48 hours.

• Takes on additional responsibilities in support of Vynca’s needs as directed by the Clinical Manager or Chief Medical Officer.

• Assists with community outreach and education about Vynca services.

• Must be able to travel to fulfill job responsibilities, attend training, or participate in company events.

⛳️ Requirements

• New Jersey LCSW, LMFT, or LPCC certification is required.

• A minimum of 2 years of experience in healthcare social work.

• Experience in hospice, palliative care, oncology, or home health is preferred.

• Palliative care certification is highly desirable.

• Strong understanding of cultural competency with the Medicaid population.

• Excellent verbal and written communication skills, along with strong interpersonal abilities.

• General computer skills and proficiency in MS Office, G-Suite, and internet navigation are necessary.

• Experience with Electronic Medical Record (EMR) systems and other business systems, such as Salesforce and Athena, is required.
