Licensed Practical Nurse – Care Advocate, Patient Navigation

This is a fully remote position, open to applicants in United States.

📋 Description

• Act as the main point of contact for patients following their intake visit.

• Assist patients in comprehending and implementing their care plan — scheduling appointments, locating in-network providers, and utilizing their Medicare benefits.

• Address navigation challenges: portals, referrals, appointment logistics, forms, and communications with providers.

• Recognize practical and social support needs (such as food, housing, transportation, etc.) and link patients to available resources.

• Collaborate with clinical team members involved in a patient's care while providing non-clinical support exclusively.

• Record every patient interaction and support activity in our platform, ensuring accuracy and timeliness.

• Uphold confidentiality and adhere to HIPAA, privacy regulations, and all relevant platform standards.

⛳️ Requirements

• Possess one of the following: an active, unrestricted LPN/LVN license in good standing.

• Have 2+ years of experience in patient- or community-facing support (such as care coordination, case management, member advocacy, health coaching, benefits navigation, community health work, or similar roles).

• Demonstrate working knowledge of the U.S. healthcare system and Medicare.

• Exhibit comfort in working independently, managing a caseload, and maintaining thorough documentation.

• Ensure access to a reliable computer, internet connection, and a quiet environment for patient calls.

• Hold experience in supporting older adults or patients with complex or serious health conditions.

• Have familiarity with Medicaid and other public assistance programs.

• Be bilingual — Spanish or another language commonly spoken by Medicare patients.
