This is a fully remote position, open to applicants in United States.
• Foster trust and develop robust relationships with our patients through telephone and virtual interactions.
• Collaborate with the interdisciplinary team (which includes clinicians, case managers, community health workers, and coordinators) to ensure integrated, patient-centered care for high-needs individuals.
• Engage with patients to uncover social barriers, facilitating resource alignment and coordinating services to enhance the effectiveness of the Complex Care Department: Conduct assessments and create tailored action plans to meet individual patient needs.
• Identify caregivers displaying signs of burnout or those actively involved in coordinating the patient's care, offering emotional support and additional resources as necessary.
• Assist in managing complex family and support dynamics.
• Lead discussions about end-of-life planning.
• Work alongside patients to pinpoint their long-term care planning requirements and aspirations. Provide information on available options and assist in making referrals and coordinating essential care.
• Oversee care coordination between the Devoted Medical Behavioral Health Team while monitoring outcomes.
• Effectively manage multiple competing priorities for complex patients.
• Cultivate relationships and coordinate efforts with key local market and internal stakeholders.
• Ability to thrive in a fast-paced environment.
• Must embrace, seek out, and feel at ease with new and constantly evolving technologies.
• Minimum of 3 years of experience in medical social work, case management, or care coordination.
• Master’s degree in social work and a valid state license.
• Bilingual in Spanish is preferred.
• Prior experience in health insurance is preferred (Medicare Advantage experience is a plus).
• Experience with the senior population is an advantage.
• Proven track record in establishing relationships with patients and working within an interdisciplinary team.
• Ability to multitask comfortably: engage in listening, conversing, evaluating, and documenting simultaneously.
• A strong team player with a positive attitude; self-reflective.
• Comfortable with remote work.
