This is a fully remote position, open to applicants in United States.
• Perform structured virtual eligibility evaluations with patients and their caregivers to verify SSI/SSDI eligibility and collect necessary information to advance applications.
• Request, compile, and review medical records from appropriate providers, and create medical summary reports for submission to the Social Security Administration (SSA) and Disability Determination Services (DDS).
• Submit finalized applications to the SSA and DDS, acting as the appointed SSA Representative for patients lacking legal representation.
• Conduct ongoing weekly outreach calls with patients to provide status updates, address inquiries, and ensure the application process remains on schedule.
• Collaborate with Patient Enrollment Specialists and Health Coaches to assist patients requiring extra engagement or care coordination touchpoints.
• Oversee administrative tasks linked to the program, including management of the PO box and documentation tracking, ensuring precision and follow-through.
• Utilize AI-assisted tools for records review and medical summary development, with a clear understanding of when to verify outputs and how to use these tools in a manner compliant with HIPAA.
• Work alongside internal teams to identify workflow gaps, share insights about patients, and contribute to the ongoing enhancement of scalable, compliant benefits enrollment processes.
• At least 2 years of experience in social work, case management, disability advocacy, or public benefit navigation, with direct involvement in supporting SSI/SSDI applications.
• Current SOAR (SSI/SSDI Outreach, Access, and Recovery) certification at the time of hiring; candidates in the process will be considered if their background is otherwise robust.
• Proven ability to effectively manage an active caseload, demonstrating a history of advancing cases at scale while upholding high-quality standards.
• Experience assisting patients with complex medical, psychological, or socioeconomic challenges.
• Strong understanding of SSA and DDS procedures, documentation requirements, and application processing guidelines.
• Proficient in virtual patient engagement, encompassing both telephone and video interactions.
• Excellent written communication skills, especially in drafting medical summary reports and case documentation.
• Knowledge of trauma-informed care, motivational interviewing, and recovery-oriented approaches.
