This is a fully remote position, open to applicants in Pennsylvania.
• Evaluate, identify, secure, and oversee services for program participants tailored to their individual needs.
• Address participant needs through various forms of communication while remaining attentive to their requirements.
• Adhere to compliance standards and timelines for assessments, service plans, visits, calls, and incident reporting.
• Document each visit and interaction in a comprehensive and person-centered manner within all required systems.
• Offer necessary training and support to participants and consumer employees in the management of direct care services.
• Maintain and utilize internal databases to monitor participant information and interactions.
• Assist participants in acquiring Accessibility Adaptations, Specialized Medical Equipment, Non-Medical Transportation, and additional services.
• Consistently exemplify company values in all interactions and responsibilities.
• Bachelor’s degree REQUIRED (preferably in social work, psychology, or related fields).
• A minimum of three years in a social service or healthcare-related environment REQUIRED.
• Experience with individuals with disabilities or seniors and/or at least one (1) year of LTSS, Service Coordination, or Case Management experience is preferred.
• Exceptional communication, customer focus, integrity, job knowledge, dependability, problem-solving, interpersonal skills, productivity, quality, self-development, and teamwork abilities.
• Proficient in utilizing computer systems and technologies relevant to the position.
• Valid driver’s license and reliable means of transportation.
