Senior LTSS Service Care Manager, RN

This is a fully remote position, open to applicants in Texas.

📋 Description

• Evaluate members in nursing facilities or at their residences following discharge.

• Create individualized care plans and service plans for members requiring long-term care.

• Oversee care plans and member status, tracking progress towards established care plan objectives.

• Analyze member data to identify patterns and enhance operational efficiency and quality of care.

• Work collaboratively with healthcare providers to coordinate member services and treatments.

⛳️ Requirements

• Graduate from an accredited nursing school.

• 4–6 years of relevant clinical nursing experience.

• Bachelor's degree in Nursing is preferred.

• RN - Registered Nurse - State Licensure and/or Compact State Licensure is required.

• Resource Utilization Group (RUG) certification must be obtained within 90 days of hire.

• Experience in clinical RN roles or case management in nursing facilities, long-term care, skilled nursing facilities, home health, or hospice settings.

• Strong critical thinking skills and the ability to use nursing judgment to evaluate health risks for members.

• Excellent clinical documentation and time management abilities.

• Flexibility, resilience, and outstanding interpersonal communication skills for advocating for members.
