Senior Integrated Clinic Pharmacist

This is a fully remote position, open to applicants in Florida.

📋 Description

• Utilize evidence-based medicine and clinical expertise to assess the appropriateness of pharmacotherapy for patients at high risk with chronic conditions such as diabetes, hypertension, heart failure, and COPD.

• Implement clinical protocols as permitted by the organization and state regulatory bodies.

• Enhance drug coverage benefits to ensure appropriate care while minimizing costs (including preferred formulary products, lower-cost alternatives, and home delivery pharmacy options).

• Collaborate with physicians, nurses, and other healthcare professionals to elevate patient care standards.

• Engage in high-risk rounds alongside a multi-disciplinary team to review patient progress and modify treatment plans as necessary.

• Demonstrate excellent patient counseling abilities to convey complex medical information effectively to both patients and healthcare providers.

• Contribute to quality improvement initiatives focused on enhancing medication safety and effectiveness.

• Record all activities within the clinic's electronic medical records (EMR) and other mandatory pharmacy or clinical management systems.

• Facilitate in-service educational sessions for healthcare staff and create and distribute educational materials related to pharmacotherapy.

⛳️ Requirements

• Possession of an active and unrestricted Pharmacist license.

• Willingness to obtain licensure in additional states as required for market support.

• Doctor of Pharmacy (Pharm.D.) or another advanced pharmacy degree.

• A minimum of 2 years of experience as a practicing pharmacist.

• Intermediate proficiency or higher in Microsoft Office applications such as Word, Excel, and PowerPoint.
