This is a fully remote position, open to applicants in Alabama, +42 more states.
• Design, develop, and sustain Healthy Planet registries, care plans, care teams, and health maintenance topics.
• Configure and enhance risk stratification tools, care gap identification, and outreach workflows within Healthy Planet.
• Lead the testing and validation processes for Healthy Planet builds.
• Create and uphold care coordination workflows for transitions of care, discharge planning, referral management, and care team communication.
• Optimize Care Everywhere and interoperability workflows.
• Collaborate with care management and case management teams to improve documentation, task management, and communication tools within Epic.
• Act as the primary Epic liaison for Population Health, Care Management, and ACO stakeholders.
• Convert program objectives into technical specifications.
• Facilitate workflow design meetings with clinical and operational leadership.
• Present build recommendations, timelines, and trade-offs to leadership and governance committees.
• Mentor junior and mid-level analysts.
• Review and validate build work prior to release.
• Contribute to documentation, standard operating procedures, and onboarding materials.
• Collaborate with reporting teams to ensure accurate dashboards and quality/outcomes reporting.
• Engage in change control, release planning, and regression testing.
• Maintain documentation regarding workflows, build configurations, and design decisions.
• Manage and refine user stories and backlog items in agile tools such as Rally.
• Bachelor's degree in Health Informatics, Information Systems, Nursing, or a related field (or equivalent experience).
• 4+ years of Epic experience, with significant hands-on involvement in Healthy Planet and/or care coordination/care management modules.
• Epic certification(s) in Healthy Planet, Care Everywhere, Ambulatory, or related modules is required or strongly preferred.
• Proven experience in designing workflows for population health, care management, or value-based care programs.
• Strong comprehension of transitions-of-care processes and cross-continuum care delivery.
• Experience in an ACO, health system, or integrated delivery network with population health responsibilities is preferred.
• Familiarity with FHIR/HL7 interoperability standards and Care Everywhere configuration is preferred.
• Experience in mentoring or informally leading junior analysts is preferred.
• Knowledge of agile delivery tools such as Rally for backlog and release management is preferred.
• Extensive Epic build expertise.
• Capability to translate clinical/operational goals into scalable technical solutions.
• Strong communication skills with stakeholders across clinical, technical, and leadership audiences.
