Senior Caseworker, Patient Support & Services

This is a fully remote position, open to applicants in Canada.

📋 Description

• Assists in managing complex case activities to coordinate patient services, facilitate resource access, and ensure continuity of support across various touchpoints.

• Organizes appointments, referrals, authorizations, and associated service needs to minimize access barriers and promote timely progression of cases.

• Addresses escalated inquiries from patients and caregivers concerning coverage, benefits, documentation, and support pathways to enhance case outcomes.

• Keeps precise case records, status updates, and supporting documentation to improve traceability, compliance, and operational transparency.

• Links patients to community resources, financial assistance options, and service programs to tackle practical barriers impacting access and adherence.

• Works in conjunction with healthcare providers, payers, pharmacies, and internal support teams to synchronize case actions and bridge service gaps.

• Participates in workflow enhancements, knowledge updates, and the adoption of standard practices to foster scalable and future-ready case operations.

⛳️ Requirements

• A High School Diploma/GED or equivalent experience is mandatory.

• An Associate’s degree in healthcare administration, health sciences, social services, or a related field is preferred.

• A minimum of 3 years of experience in case management, patient support services, customer operations, healthcare coordination, social services, or a related field is required.

• Certification in case management, patient services, healthcare support, or customer service operations is preferred.
