Senior Care Coach

This is a fully remote position, open to applicants in Illinois.

📋 Description

• Assist the manager by providing guidance, coaching, and mentoring to team members.

• Support, guide, and enhance team processes while fostering a positive work atmosphere.

• Offer fieldwork assistance and oversee fieldwork visits conducted by team members.

• Elevate issues to the manager and aid in team management as needed.

• Cover for Care Coordinators 1 and 2 and ensure documentation in the clinical system.

• Mentor the team and facilitate training initiatives.

• Deliver daily reports on outstanding cases and assess daily compliance dashboards.

• Assist with audit initiatives and manage both internal and external auditing processes.

• Conduct fieldwork support and evaluate reports.

• Facilitate new-hire onboarding, encompassing precepting and case audits.

• Coordinate services while monitoring and assessing case management plans against members’ personal goals.

• Oversee patient care through assessments, data analysis, member conversations, and proactive care planning.

• Direct members and families to appropriate care resources.

• Foster interactions with payer sources, providers, interdisciplinary teams, and other stakeholders involved in members’ care.

• Conduct home visits for Medicaid members as well as visits in long-term care facilities and other care environments.

• Provide support for either MLTSS or LTSS based on business requirements.

⛳️ Requirements

• Bachelor's degree in social sciences, social work, human services, or a related field, OR an unrestricted Illinois LPN with one year of experience in conducting comprehensive assessments and delivering formal services for the elderly, OR a Licensed Professional Counselor (LPC), OR a Licensed Social Worker (LSW).

• Must be a resident of the State of Illinois.

• A minimum of 2 years of experience in health care and/or case management.

• At least 2 years of experience working with Medicare and Medicaid recipients, long-term care, Home and Community Based Services, and/or managed care organizations.

• Proficiency in using electronic information applications and software, including electronic medical records.

• Completion of tuberculosis screening is required if selected.

• Valid state driver’s license is necessary.

• Proof of personal vehicle liability insurance with coverage limits of at least 100,000/300,000/100,000.

• This is a patient-facing role subject to Humana’s TB screening program.
