RN Healthcare Advocate

This is a fully remote position, open to applicants in United States.

📋 Description

• Establish trustworthy relationships with senior adults and their families, rooted in active listening and empathy.

• Act as a primary resource for navigating medical systems, social services, and emotional wellness requirements.

• Develop personalized care plans that cater to both clinical necessities and Social Determinants of Health (SDOH), such as:

• Access to food and housing assistance programs

• Coordination of transportation

• Management and reminders for medications

• Scheduling appointments and ensuring follow-ups

• Support and educate caregivers

• Work collaboratively with Sailor’s therapists, care coordinators, and technological platform to provide integrated, person-centered care.

• Offer insights to help refine Sailor’s patient advocacy model as we expand.

⛳️ Requirements

• Active Registered Nurse license in good standing

• Experience in healthcare advocacy or care navigation, specifically with Medicare populations

• Understanding of SDOH and their impact on patient outcomes

• Ability to exercise clinical judgment while demonstrating empathy and problem-solving skills

• Highly organized, communicative, and capable of managing multiple patients and varying needs

• Proficient with technology—you are comfortable with learning new systems and working remotely

• Proactive and motivated by the opportunity to build and innovate

• Located in the U.S.
