• Provide clinical oversight and support to guarantee effective care management for members within the ECM program.
• Work collaboratively with Lead Care Managers (LCMs) and additional clinical staff (e.g., RNs, LCSWs, RDs) to address member care requirements and facilitate clinical decision-making.
• Assist in gaining access to necessary healthcare services, including primary care, behavioral health, and specialty services as required.
• Oversee the accuracy and alignment of Comprehensive Assessments and Care Management Plans with healthcare plan guidelines.
• Lead and/or engage in Emergent and Non-Emergent Clinical Case Conferences to tackle complex member needs and ensure prompt interventions.
• Contribute to Interdisciplinary Care Team (ICT) meetings by offering clinical insights and recommendations to enhance coordinated, whole-person care.
• Conduct and/or supervise medication reconciliation to ensure the safety and precision of member medication regimens.
• Support and facilitate smooth Transitions of Care (TOC) post-emergency room visits and hospitalizations, ensuring timely follow-up and continuity of care.
• Engage members, as suitable, utilizing clinical expertise and motivational interviewing techniques to encourage health behavior changes and adherence to care plans.
• Deliver health education within the scope of practice to promote better health outcomes.
• Coordinate care through collaboration with internal teams and external providers, ensuring uninterrupted communication and continuity of care.
• Ensure that all clinical activities are documented accurately and comply with organizational, regulatory, and healthcare plan requirements.
• Consult and collaborate with clinical leadership as necessary for complex cases or escalations.
• Participate in case reviews and clinical discussions to uphold quality, consistency, and ongoing improvement.
• Comply with all departmental and organizational policies, procedures, and compliance requirements.
• Perform additional duties and projects as assigned.
• Associate’s degree in nursing.
• At least 1+ years of experience as an RN in a healthcare environment, such as a medical clinic or acute care hospital.
• A current and unrestricted RN license in California.
• Proficiency in using an EHR/EPM.
• A distraction-free home workspace equipped with a secure internet connection.
• Preferred experience in care management.
• Bachelor’s degree in nursing.
• Familiarity with eClinical Works (eCW).
• Valid BLS certification from the American Heart Association.
• Valid CPR certification.
• Bilingual in Spanish.
