RN Case Manager – IKC National Flex Team

This is a fully remote position, open to applicants in Arizona, +3 more states.

📋 Description

• This role operates both independently and collaboratively with all members of the healthcare team to coordinate and deliver quality, cost-effective care while reducing fragmentation in the healthcare delivery system for patients with ESRD and CKD.

• This role is responsible for coordinating care among the patient, family, physician, provider, care teams, the community, and the Regional Operations Manager.

⛳️ Requirements

• An A.D.N degree from an accredited nursing school is required; a B.S.N is preferred; a three-year diploma from an accredited diploma program may be accepted in lieu of a nursing degree.

• A minimum of 1 year of IKC experience as a DaVita IKC RN Case Manager is required.

• A Compact Nursing License is highly preferred.

• A minimum of five (5) years of clinical nursing experience is required.

• A minimum of three (3) years of renal nursing experience is preferred.

• Current CPR certification is mandatory.

• Basic computer skills and proficiency in MS Word and Outlook are essential.

• Functional proficiency with DaVita-specific clinical software programs, including Capella, must be achieved within 90 days of employment.
