RN Case Manager – Behavioral Health

This is a fully remote position, open to applicants in Illinois.

📋 Description

• Establish significant relationships with patients and their families, gaining insight into their aspirations, challenges, and motivators.

• Develop tailored care plans that address medical diagnoses, social determinants of health, and emotional wellness.

• Facilitate care coordination among various providers and settings to guarantee safety, quality, and continuity of care.

• Educate and empower members to make well-informed healthcare choices and navigate the healthcare landscape effectively.

• Collaborate with a multidisciplinary team and advocate for comprehensive health approaches.

• Utilize data to pinpoint opportunities for improvement and assess impact while keeping patients as the focal point.

• Ensure adherence to regulatory standards while emphasizing compassion and connection.

• Adapt to changing business requirements and support initiatives across different functions.

• Engage in occasional travel, which may be up to 10% as necessary.

⛳️ Requirements

• Valid Illinois RN license.

• Robust experience in care management, particularly with complex and chronic conditions.

• Proficient in motivational interviewing, patient education, and advocating for patients.

• Capability to work autonomously in a remote setting while maintaining team connectivity.

• Comfortable using technology and various EMR systems.

• At least five (5) years of clinical experience in varied healthcare environments.

• In-depth understanding of psychiatric conditions, crisis intervention, and effective therapeutic communication.

• Ability to maintain compliance with regulatory standards.

• Flexibility to work during designated hours and remain available to the team.

• Reliable internet connection suitable for video conferencing and system access, with a minimum of 100 Mbps download and 10 Mbps upload speed.
