RN Case Manager

This is a fully remote position, open to applicants in California.

📋 Description

• Conduct thorough clinical evaluations and pinpoint medical, behavioral, and social obstacles to health.

• Create, execute, and track personalized care plans for high-risk individuals.

• Oversee a cohort of members with intricate healthcare requirements across various settings and providers.

• Facilitate care transitions following visits to emergency departments, hospital admissions, and skilled nursing facilities.

• Work closely with Community Health Workers to guarantee that members receive both clinical and community support.

• Recognize care gaps and take proactive measures to avert unnecessary utilization and detrimental outcomes.

• Offer health coaching, education on disease management, medication assistance, and self-management training.

• Engage in interdisciplinary care team meetings and case evaluations.

• Ensure precise documentation and adherence to regulatory standards related to ECM and associated programs.

• Assist in achieving quality, utilization, and member outcome objectives set by the organization and health plan partners.

⛳️ Requirements

• Active Registered Nurse license in California.

• Minimum of 3 years of nursing experience in care management, case management, population health, home health, primary care, managed care, or similar environments.

• Background in managing medically intricate and high-risk populations.

• Strong grasp of care coordination, utilization management, transitions of care, and chronic disease management.

• Outstanding communication and relationship-building abilities.

• Capacity to work efficiently in interdisciplinary and remote care settings.

• Required: Proficiency in both English and Spanish, including effective verbal and written communication skills.
