RN Case Manager

This is a fully remote position, open to applicants in Louisiana.

📋 Description

• Establishing meaningful connections with patients and their families, while understanding their objectives, challenges, and motivations.

• Developing tailored care plans that extend beyond mere diagnoses, focusing on social determinants and emotional well-being.

• Coordinating care across various providers and settings to ensure safety, quality, and continuity of care.

• Educating and empowering members to make informed choices and navigate the healthcare system with confidence.

• Collaborating with an interdisciplinary team to share insights and advocate for comprehensive health approaches.

• Utilizing data as an insightful resource—not as the sole driver of care—to identify opportunities and measure impact, always placing the patient at the forefront.

• Ensuring adherence to regulatory standards while emphasizing compassion and connection.

⛳️ Requirements

• A minimum of 5 years of experience as a Registered Nurse.

• An active Compact RN License.

• Strong experience in care management, particularly in managing complex and chronic conditions.

• Proficiency in motivational interviewing, patient education, and advocacy for patients.

• Ability to work autonomously in a remote setting while maintaining team connectivity.

• Comfort with technology and various electronic medical record (EMR) systems.

• Must reside in St. Tammany Parish, LA.
