This is a fully remote position, open to applicants in United States.

đź“‹ Description

• Evaluates, plans, implements, coordinates, monitors, and reviews options and services necessary to address an individual’s health requirements by utilizing plan benefits and community resources to ensure effective cost management and containment measures are achieved.

• Performs additional responsibilities as assigned.

• Engages in nursing practice within the confines of licensure, complying with policies, procedures, regulations, URAC standards, and state-specific regulations, making informed decisions to ensure adherence, appropriate care levels, and patient safety.

• Delivers specialized education and expertise on disease-specific conditions as required and assigned.

• Stays informed about the latest medical and surgical procedures, products, healthcare services and medications, as well as general trends in healthcare delivery, enterprise procedures, policies, and contracts.

• Acts as a liaison to facilitate a collaborative process involving patients, families/caregivers, physicians, hospital discharge planners, home care providers, and other ancillary providers to enhance care quality, cost efficiency, accessibility, and appropriateness of service levels.

• Proactively collaborates with referrals from various sources to identify suitable candidates for case management, clarifies services ensuring voluntary consent, and maintains a manageable patient caseload in a timely manner.


⛳️ Requirements

• Preferred Bachelor's degree in Nursing.

• Current active state license as a Registered Nurse (RN) in good standing in the state(s) where job duties are executed is required.

• Case management certification is mandatory.

• A minimum of four (4) years of clinical practice experience in at least one of the following areas: case management, home health, critical care, medical/surgical, discharge planning, concurrent review, or obstetric/neonatal care.
