RN Care Manager – Oncology

This is a fully remote position, open to applicants in Pennsylvania.

📋 Description

• Accountable for coordinating services for members who meet specified criteria, focusing on education/self-management while promoting quality care and cost-effective outcomes.

• Engage in a collaborative process to evaluate member needs, explore options for services and resources, create and execute a care plan, coordinate resources, monitor progress, assess member status, discharge members, and thoroughly document the personalized member care management process and results.

• Address medical, psychosocial, clinical needs, and behavioral health requirements, including those of members with mental health and substance use disorders.

• Offer counseling and referrals to community, local, and state programs; collaborate with caregivers, providers, community agencies, behavioral health specialists, and other resources to coordinate care aimed at achieving the member's optimal health status.

• Oversee and manage a caseload of members at moderate to high risk with complex medical, behavioral, and psychosocial needs.

⛳️ Requirements

• A minimum of three (3) years of recent and relevant experience; care management, home health, medical/surgical behavioral health, and/or critical care experience is preferred.

• Preferred experience in health plan operations.

• Strong preference for experience related to Oncology.

• Experience in health coaching and motivational interviewing techniques is preferred.

• Registered Nurse with active licensure in the home state; additional state licensure may be required to meet customer needs.

• Certified Case Management certification must be obtained within two years of employment.
