RN Care Manager

This is a fully remote position, open to applicants in Louisiana.

📋 Description

• Create personalized care plans that prioritize goals, taking into account the preferences and desired involvement of the patient and caregivers in the management process.

• Conduct outbound calls to evaluate the patient's current health condition.

• Recognize gaps or obstacles in treatment strategies.

• Facilitate care coordination for members and make referrals to external resources.

• Ensure that members transitioning from the hospital or emergency department receive necessary services and resources, including medication reconciliation.

• Triage calls based on symptoms according to approved protocols for a diverse range of patients.

• Enroll patients and oversee alerts for various home monitoring programs.

• Provide approved general health information and guidance to public callers by assessing their needs.

• Record all workflows in the EPIC system for tracking and quality assessment purposes.

• Educate patients to support self-management of wellness and disease.

• Inform enrolled members about chronic and acute disease processes.

• Collaborate with LCMC Health care partners, leadership, and/or physicians to address clinical questions, concerns, strategies, and care plans aimed at achieving quality and cost management goals.

• Exhibit enthusiasm, energy, and a commitment to delivering quality work.

⛳️ Requirements

• A valid nursing license to practice in Louisiana, as specified by the Louisiana State Board of Nursing.

• A minimum of 2 years of professional nursing or care management experience.

• Preferred experience in Utilization Review and/or Care Management.

• Required acute hospital experience.

• Familiarity with Epic & InterQual is preferred.
