📋 Description

• Oversee a substantial panel of Medicare patients dealing with chronic conditions

• Conduct monthly clinical calls that lead to tangible outcomes and minimize hospital admissions

• Facilitate monthly clinical discussions with patients in chronic care

• Encourage behavior modification through coaching and educational initiatives

• Ensure accurate and compliant documentation within our platform

• Address preventive care gaps and coordinate necessary services

• Revise care plans and monitor interventions

• Aid in transitions of care to lower readmission rates

• Manage your panel with efficiency and consistency

⛳️ Requirements

• Active unrestricted Compact RN license

• Minimum of 3 years of clinical experience

• Reliable internet connection and a dedicated workspace

• Proficient in using various software platforms

• Excellent communication and critical thinking abilities

• Licensed as a Compact RN

• Highly self-motivated

• Comfortable managing a large caseload (over 100 patients per month)

• Highly organized and driven by metrics

• Tech-savvy and quick to learn new tools

• Motivated by performance and outcomes

• Able to work independently across various practices, workflows, and EHR systems

• Accountable, disciplined, and dependable
