This is a fully remote position, open to applicants in United States.
• Act as a clinical resource for the cancer screening and prevention population by providing triage, care coordination, and education through phone and asynchronous messaging.
• Conduct clinical assessments and determine patient needs.
• Carry out telephone consultations for patients who report symptoms.
• Coordinate follow-up care and escalate cases to the appropriate level of care when necessary.
• Follow up on abnormal cancer screening results, educate patients on subsequent steps, and arrange follow-up appointments until the completion of workups.
• Review annual patient engagement surveys to identify changes in screening and clinical needs.
• Deliver compassionate, empathetic, and trust-based patient care.
• Collaborate with Care Advocates to address non-clinical support and care requirements.
• Develop clinical protocols and best practices for effective care coordination from screening through survivorship.
• Engage with patients regarding symptom management, care escalations, provider referrals, and in-person care planning.
• Participate in care team sessions to track member progress and create targeted support plans.
• Maintain comprehensive and detailed documentation of encounters.
• Partner with physicians, advanced practice providers, genetic counselors, and care advocates.
• A Bachelor of Science in Nursing (BSN) is required.
• An unrestricted active Registered Nurse (RN) license, with the ability to obtain additional state licenses as necessary.
• A related certification is required or must be obtained within 12 months of hiring, such as AMB-C, CCM, CMGT-BC™, OCN, AOCN, or AOCNS.
• A minimum of 7 years of nursing experience is necessary.
• At least 5 years of experience in primary care with follow-up result management.
• Outpatient experience is required.
• Inpatient and/or oncology experience is advantageous.
• Exceptional organizational and multitasking abilities.
• Strong verbal and written communication skills, demonstrating empathy and active listening.
• Proficiency with video conferencing, electronic health records, and other telehealth technologies.
• Professional fluency in both English and Spanish, including medical Spanish, is strongly preferred.
• Regular availability during business hours in Pacific Time.
