Rehabilitation Service Specialist – Atlantic

This is a fully remote position, open to applicants in Canada.

📋 Description

• Assess the necessary rehabilitation evaluations to better understand the Participant’s potential for improvement and outline their rehabilitation requirements, coordinating as needed.

• Ensure that services and plans are tailored to individual needs, focused on goals, and compliant with program mandates aimed at enhancing the six functional domains of well-being while minimizing barriers to re-establishment, ensuring safety, cost-effectiveness, and high quality.

• Create a comprehensive Rehabilitation Plan for program Participants in collaboration with the Participant, their family/caregiver, Rehabilitation Service Professionals (RSPs), and the Veterans Affairs Canada (VAC) Case Manager.

• Serve as the service integrator, working in partnership with medical, psycho-social, and vocational RSPs to deliver services to Participants.

• Empower Participants to find solutions by examining community-based interventions and resources to attain their desired objectives.

• Recognize barriers or risk factors that may lead to less-than-optimal outcomes and proactively devise and implement strategies.

⛳️ Requirements

• A bachelor’s degree in Psychology, Counselling, Rehabilitation, Social Sciences, Health Services, Human Services, or a related discipline.

• One of the following: a health professional designation; or Certified Vocational Rehabilitation Professional (CVRP or CVRP-F).

• At least three (3) years of experience in directly providing medical, psycho-social, or vocational rehabilitation services (or a combination thereof).

• Experience working with individuals facing mental or physical health challenges that create barriers in their lives.

• Employment is contingent upon obtaining federal security clearance – Reliability Status.

• Willingness to travel across the region as necessary.
