This is a fully remote position, open to applicants in United States.
• Evaluate clients and create individualized care plans.
• Revise care plans as client conditions evolve.
• Perform supervisory visits to assess caregiver performance and adherence to care plans.
• Provide training and support to caregivers.
• Record all assessments, updates, and visits meticulously.
• Liaise with clients, families, and staff to facilitate coordinated care.
• Ensure adherence to regulations and standards.
• Engage in quality assurance activities.
• Manage care plans and assist in caregiver supervision to maintain a high standard of care.
• Active and current Registered Nurse (RN) license in { job.state }.
• At least 1–2 years of nursing experience, preferably in home care or geriatric care.
• Proficient assessment and documentation abilities.
• Exceptional communication and interpersonal skills.
• Capability to supervise and support non-clinical staff.
• Detail-oriented with strong organizational and time management capabilities.
• Valid driver's license and dependable transportation for home visits.
• Knowledge of state home care regulations and best practices.
