
Registered Nurse – Remote Patient Monitoring, Chronic Care Management

This is a fully remote position, open to applicants in South Carolina.
• Greet patients entering continuous care program(s) and review the benefits and services provided.
• Collaborate with the patient's Respiratory Therapist and pulmonary providers to ensure a unified approach to care.
• Educate patients on the usage and frequency of their assigned in-home monitoring devices, if applicable.
• Develop a personalized, comprehensive care plan with the patient through phone or video consultations.
• Identify and resolve any obstacles to patient success.
• Offer targeted education and coaching on patients' chronic pulmonary conditions.
• Maintain regular contact with the patient to review readings and update their care plan on a monthly basis.
• Act as the patient's primary contact for all non-emergent inquiries (e.g., medication refills, scheduling, appointment reminders, etc.).
• Assist patients in navigating their healthcare (e.g., helping with appointment scheduling, coordinating with specialists, and ensuring completion of preventative screenings).
• Monitor and assess in-home device readings in real-time during standard business hours.
• Adhere to established protocols for identifying, communicating, and documenting device trends and any related symptoms in the patient's medical record.
• Elevate concerning readings and/or symptoms to the provider in accordance with a communication protocol established by the provider.
• Build a meaningful rapport that fosters trust, open communication, and motivation for positive changes in the patient's health.
• Active and Unrestricted Georgia RN License (Compact License preferred).
• Minimum of three (3) years of experience in adult health preferred.
• Background in adult chronic health conditions (Pulmonology, Cardiology, Care Management) is preferred.
• Experience with a variety of Electronic Medical Record (EMR) systems.
• Strong knowledge, skills, and interest in fundamental computer skills.
• Proficient in problem-solving and capable of multitasking.
• Exceptional communication skills (both oral and written).
• Strong teamwork abilities.
• Clean background check and drug screening.
• Comfortable working remotely while fostering collaboration.
