Registered Nurse – Care Coach

This is a fully remote position, open to applicants in North Carolina, +1 more state.

📋 Description

• Employ our specialized care management software to reach out to Medicare patients who have two or more chronic conditions (such as Diabetes, CHF, Chronic Pain, COPD, etc.) on a monthly basis.

• Establish and nurture relationships with patients to guide them toward better health through SMART goals and education on self-management techniques.

• Enhance and update the Plan of Care by adjusting medications, tracking due appointments, monitoring biometrics, documenting symptoms, and noting interventions.

• Link patients with community resources as needed, including transportation, personal care assistance, prescription/DME support, social services, and more.

• Conduct Transitional Care Management activities for high-risk patients discharged from hospitals and emergency rooms to minimize unnecessary readmissions.

• Address care gaps by promoting and facilitating preventive healthcare measures, such as annual wellness visits, vaccinations, cancer screenings, and follow-up appointments with specialists.

• Proficient in English.

• Self-motivated and capable of working independently with minimal supervision while achieving performance goals.

• Strong commitment to nursing and enhancing patient outcomes.

• Technologically adept and enthusiastic about learning and utilizing new software.

• Excellent organizational skills and effective time management.

• Timely communication is crucial; nurses are expected to reply to all messages and emails within 24–48 hours.

• Strong critical thinking and problem-solving abilities.

• Current, unrestricted Compact License / multistate RN license.

• Proficient in using electronic health records and web-based applications.

• A minimum of 3 years of experience as a Registered Nurse.

🏝️ Benefits

• RN Care Coach compensation is set at $15.00 for the first 20-minute increment (20-min milestone).

• $12 for the second 20-minute increment (40-min milestone).

• $11.50 for the third 20-minute increment (60-min milestone).

• Payments are made monthly via direct deposit, 40 days after the last day of the month of service.
