Registered Nurse, Care Advocate – Patient Navigation

This is a fully remote position, open to applicants in United States.

📋 Description

• Act as the main point of contact for patients following their intake visit.

• Assist patients in comprehending and implementing their care plan.

• Address and resolve any navigation challenges.

• Identify support for practical and social needs.

• Collaborate with clinical team members involved in a patient's care.

• Accurately document each patient interaction in a timely manner.

⛳️ Requirements

• Current, unrestricted RN license in good standing.

• Minimum of 3 years of nursing or patient-facing experience.

• Familiarity with the U.S. healthcare system and Medicare.

• Access to a reliable computer, internet, and a quiet environment for patient calls.
