This is a fully remote position, open to applicants in Connecticut, +13 more states.
• Provide exceptional, patient-focused Medical Nutrition Therapy (MNT) for individuals dealing with chronic and complex health issues such as diabetes, cardiovascular disease, kidney disease, obesity, cancer, and gastrointestinal disorders.
• Conduct thorough virtual assessments and create personalized nutrition care plans tailored to the specific needs, preferences, and goals of each patient.
• Develop nutrition prescriptions that align with clinical diagnoses and encourage sustainable behavior modifications.
• Accurately document all patient interactions in the electronic health record (EHR) in a timely and clear manner.
• Work collaboratively with care teams, including physicians and health coaches, to enhance coordinated patient care.
• Engage in content creation for educational initiatives, patient engagement, and outreach efforts.
• Participate in continuous quality improvement and innovate care delivery methods.
• Implement current nutrition research and evidence-based practices in patient care.
• Uphold all necessary professional credentials, licensing, and ongoing education requirements.
• Join cross-functional meetings and provide input on strategic projects as needed.
• Must possess a current Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) credential from the Commission on Dietetic Registration, in good standing, along with a Master’s Degree in Nutrition or a related field preferred; OR a Bachelor's Degree with RD/RDN status obtained before the Master's degree requirement takes effect (January 1, 2024).

• A passion for innovation, public health, and broadening access to nutrition care.
• Bonus qualifications include: Credentialing with major health plans, active licensure in multiple states, experience in behavior change counseling, group education, or culinary nutrition, comfort in contributing to program design, content development, or digital tools, and fluency in Spanish.
