This is a fully remote position, open to applicants in Massachusetts.
• Provide high-quality, patient-focused Medical Nutrition Therapy (MNT) for individuals with chronic and complex conditions such as diabetes, cardiovascular disease, kidney disease, obesity, cancer, and gastrointestinal disorders.
• Conduct thorough virtual assessments and create personalized nutrition care plans based on the specific needs, preferences, and goals of each patient.
• Formulate nutrition prescriptions aligned with clinical diagnoses and encourage sustainable behavioral changes.
• Accurately document all patient interactions in the electronic health record (EHR) in a timely and clear manner.
• Collaborate with care teams, including physicians and health coaches, to facilitate coordinated patient care.
• Engage in the development of content for education, engagement, and outreach initiatives.
• Participate in ongoing quality improvement efforts and innovations in care delivery.
• Integrate current nutrition research and evidence-based practices into patient care.
• Maintain necessary professional credentials, licenses, and pursue ongoing education.
• Join cross-functional meetings and contribute to strategic projects as appropriate.
• Current Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) credentialed by the Commission on Dietetic Registration, in good standing, AND a Master’s Degree in Nutrition or a related field preferred; OR a Bachelor's Degree with RD/RDN status obtained prior to the implementation of the Master's degree requirement (January 1, 2024).
• Licensed to practice in the state of Massachusetts.
• Exceptional communication skills and a strong dedication to patient-centered care.
• Proficiency in utilizing digital tools and platforms for virtual care delivery.
• Ability to effectively serve diverse patient populations and address various health conditions.
• Strong problem-solving, critical thinking, and organizational abilities.
• A proactive, collaborative mindset with the ability to navigate ambiguity in a fast-paced setting.
• A deep passion for innovation, public health, and enhancing access to nutrition care.
