This is a fully remote position, open to applicants in Maryland.
• Provide exceptional, patient-focused Medical Nutrition Therapy (MNT) for individuals with chronic and complex conditions such as diabetes, cardiovascular disease, kidney disease, obesity, cancer, and gastrointestinal disorders.
• Conduct thorough virtual assessments and create personalized nutrition care plans that align with patient needs, preferences, and objectives.
• Develop nutrition prescriptions customized to clinical diagnoses and encourage sustainable behavioral changes.
• Accurately document all patient interactions in the electronic health record (EHR) in a timely manner.
• Collaborate with care teams, including physicians and health coaches, to facilitate coordinated patient care.
• Engage in the development of content for education, outreach, and patient engagement initiatives.
• Participate in ongoing quality improvement efforts and innovate care delivery methods.
• Utilize contemporary nutrition research and evidence-based practices in the provision of patient care.
• Maintain necessary professional credentials, licensure, and commitment to ongoing education.
• Take part in cross-functional meetings and contribute to strategic projects as appropriate.
• Current Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) credentialed by the Commission on Dietetic Registration, in good standing, AND a Master’s Degree in Nutrition or a related field preferred; OR a Bachelor's Degree with RD/RDN status achieved prior to the implementation of the Master's degree requirement (January 1, 2024).
• Must be licensed in the state of Maryland.
