This is a fully remote position, open to applicants in Illinois.
• Provide exceptional, patient-focused Medical Nutrition Therapy (MNT) for individuals facing chronic and complex health issues, such as diabetes, cardiovascular conditions, kidney disease, obesity, cancer, and gastrointestinal disorders.
• Conduct thorough virtual assessments and create personalized nutrition care plans that align with the unique needs, preferences, and goals of each patient.
• Draft nutrition prescriptions that are tailored to specific clinical diagnoses and encourage lasting behavior changes.
• Accurately document all patient interactions in the electronic health record (EHR) in a timely and clear manner.
• Collaborate with multidisciplinary care teams, including physicians and health coaches, to enhance coordinated patient care.
• Engage in the development of educational content for outreach and patient engagement initiatives.
• Participate in ongoing quality improvement efforts and innovations in care delivery.
• Utilize the latest nutrition research and evidence-based practices in patient treatment.
• Keep all necessary professional credentials, licenses, and continuing education up to date.
• Take part in cross-departmental meetings and contribute to strategic initiatives as appropriate.
• Must possess a current Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) credential from the Commission on Dietetic Registration, in good standing, along with a Master’s Degree in Nutrition or a related field preferred; OR a Bachelor's Degree with RD/RDN status obtained before the Master’s degree requirement takes effect (January 1, 2024).
• Must be licensed to practice in the state of Illinois.
