This is a fully remote position, open to applicants in United States.
• Provide exceptional, patient-focused Medical Nutrition Therapy (MNT) for individuals with chronic and complex conditions such as diabetes, cardiovascular disease, kidney disease, obesity, cancer, and gastrointestinal disorders.
• Conduct thorough virtual assessments and create personalized nutrition care plans tailored to patient needs, preferences, and objectives.
• Develop nutrition prescriptions aligned with clinical diagnoses and encourage sustainable lifestyle changes.
• Record all patient interactions promptly and clearly in the electronic health record (EHR).
• Collaborate with multidisciplinary care teams, including physicians and health coaches, to facilitate coordinated patient care.
• Contribute to the creation of content for education, engagement, and outreach initiatives.
• Engage in ongoing quality improvement and innovation in care delivery practices.
• Implement current nutrition research and evidence-based methodologies in patient care.
• Uphold necessary professional credentials, licenses, and continuing education requirements.
• Attend cross-functional meetings and participate in strategic projects as appropriate.
• Current Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) credentialed by the Commission on Dietetic Registration, in good standing, along with a preferred Master’s Degree in Nutrition or a related field; OR a Bachelor's Degree with RD/RDN status obtained before the Master’s degree requirement takes effect (January 1, 2024).
• Licensed to practice in the state of Georgia.
