This is a fully remote position, open to applicants in Florida.
• Provide exceptional, person-focused Medical Nutrition Therapy (MNT) for patients dealing with chronic and complex health conditions, including diabetes, cardiovascular issues, kidney disease, obesity, cancer, and gastrointestinal disorders.
• Conduct thorough virtual assessments and create personalized nutrition care plans that cater to patient needs, preferences, and objectives.
• Generate nutrition prescriptions that align with clinical diagnoses and encourage sustainable lifestyle changes.
• Accurately document all patient interactions in the electronic health record (EHR) in a timely manner.
• Work collaboratively with healthcare teams, including physicians and health coaches, to ensure coordinated patient care.
• Engage in content creation for education, outreach, and patient engagement initiatives.
• Participate in ongoing quality improvement efforts and innovate care delivery methods.
• Utilize the latest nutrition research and evidence-based practices in patient care.
• Uphold all necessary professional credentials, licenses, and continuous education requirements.
• Take part in cross-functional meetings and contribute to strategic projects as appropriate.
• Must possess a current Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) credential from the Commission on Dietetic Registration, in good standing, along with a preferred Master’s Degree in Nutrition or a related field; OR a Bachelor's Degree with RD/RDN status achieved before the Master's degree requirement takes effect (January 1, 2024).
• Must hold a valid license in the state of Florida.
