This is a fully remote position, open to applicants in Connecticut.
• Provide high-quality, patient-centered Medical Nutrition Therapy (MNT) for individuals with chronic and complex conditions such as diabetes, cardiovascular disease, kidney disease, obesity, cancer, and gastrointestinal disorders.
• Conduct thorough virtual assessments and create personalized nutrition care plans that align with patient needs, preferences, and objectives.
• Develop nutritional prescriptions tailored to clinical diagnoses and encourage sustainable behavior changes.
• Accurately document all patient interactions in the electronic health record (EHR) in a timely manner.
• Collaborate with multidisciplinary care teams, including physicians and health coaches, to facilitate coordinated patient care.
• Engage in the development of content for education, outreach, and patient engagement.
• Participate in ongoing quality improvement initiatives and innovate care delivery methods.
• Utilize current nutrition research and evidence-based practices in patient management.
• Maintain necessary professional credentials, licenses, and engage in continual education.
• Join cross-functional meetings and contribute to strategic initiatives as needed.
• Active Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN) credentials issued by the Commission on Dietetic Registration, in good standing AND a Master’s Degree in Nutrition or a related discipline preferred; OR a Bachelor’s Degree with RD/RDN status obtained before the Master’s degree requirement takes effect (January 1, 2024).
• Licensed to practice in the state of Connecticut.
