This is a fully remote position, open to applicants in United States.
• Collaborate with a multidisciplinary team of professionals, including nurses, social workers, care coordinators, and others, to provide comprehensive virtual care services to patients.
• Perform virtual nutritional evaluations for patients with chronic kidney disease (CKD) and additional co-existing conditions, utilizing available patient information, self-reported health data, and dietary intake to inform care recommendations.
• Create individualized, evidence-based nutrition care plans and calculate nutritional requirements tailored to the needs and preferences of patients across different CKD stages, often considering other comorbid dietary factors, with a focus on dietary and lifestyle changes that promote holistic disease management.
• Educate patients and their caregivers on dietary and lifestyle modifications related to kidney health, including nutritional aspects associated with kidney disease and co-existing conditions.
• Provide patients with resources and tools to help manage their health conditions effectively.
• Keep detailed and timely records of all patient interactions, care plans, and interventions, ensuring adherence to company policies and regulatory standards.
• Operate within the registered dietitian scope of practice, making necessary referrals to nurses, social workers, care coordinators, and physicians when appropriate.
• Bachelor’s Degree in Nutrition, Dietetics, or a related field from an accredited institution.
• Must possess an active unrestricted Registered Dietitian certification granted by the Commission on Dietetic Registration.
• Minimum of relevant state dietitian licensure or registration; willingness to obtain and maintain additional state dietitian licensures as required.
• Over 2 years of experience providing care to patients with kidney disease or other chronic conditions.
• Strong understanding of healthcare regulations (including HIPAA) and a commitment to patient confidentiality, ethics, and professional standards.
• Proficient in utilizing virtual care platforms for education delivery, documentation of care, interdisciplinary collaboration, and creating a positive, engaging, and impactful remote patient experience.
• Skilled in patient counseling and education, including assessing understanding, navigating sensitive discussions, and addressing health concerns.
• Ability to modify care strategies and effectively problem-solve to meet patients’ evolving needs in a virtual or remote environment.
• Experience in developing personalized nutrition guidance that fosters patient self-management, prevention, and sustainable behavior change.
• Excellent communication skills, with the capacity to convey complex nutrition concepts into clear, practical, and compassionate advice for patients and caregivers.
