Reentry Care Manager

This is a fully remote position, open to applicants in Washington.

📋 Description

• Overseeing care coordination efforts for participants within a Managed Care environment.

• Designing, evaluating, and facilitating intricate care management initiatives primarily focused on mental and behavioral health needs.

• Assessing member requirements through phone consultations or in-home visits.

• Creating ongoing care plans for members with high acuity levels.

• Facilitating the connection between members and community resources to ensure appropriate care.

• Monitoring care plans and service plans, offering recommendations based on the identified needs of members.

• Gathering, documenting, and preserving member information alongside care management activities.

⛳️ Requirements

• A Master's degree in Behavioral Health or Social Work, or a degree from an accredited Nursing School.

• 2 to 4 years of relevant experience.

• Licensed Master's Behavioral Health Professional (e.g., LCSW, LMSW, LMFT, LMHC, LPC) or RN, in accordance with state contract requirements and with experience in Behavioral Health.
