Radiology Care Coordination Specialist

This is a fully remote position, open to applicants in Michigan.

📋 Description

• Responsible for coordinating the next level of service, care, and needs for patients with incidental findings and lung cancer screenings identified in radiology exams through the daily tracking board.

• Serves as a resource for patients, families, or physicians seeking assistance, collaborating effectively with the FIND/LCS program Nurse Navigators.

• Monitors and documents interventions and outcomes.

• Ensures the collection of appropriate data to assess program interventions and outcomes.

• Facilitates the scheduling and referral of patients to specialty providers within the healthcare system.

• Prepares patients for appointments by providing clear and understandable information, along with written follow-up in the patient's preferred format (text, email, mail, etc.).

• Acts as a direct contact and resource for patients and families, offering emotional support and educational resources.

• Reviews radiology reports.

• Compiles and assesses information within reports to assist in following algorithms, generating documents, and formalizing processes for transitions in commonly followed care pathways.

• Participates in care conferences, tumor boards, and other necessary meetings.

• Collaborates with program stakeholders to support data collection for registry participation and quality reporting.

• Engages in performance improvement activities as determined by outcomes related to program efficiency, patient experience, and results.

• Maintains positive rapport and cooperative relationships.

• Approaches conflicts constructively.

• Aids in identifying problems, proposing solutions, and participating in their resolution.

• Upholds the confidentiality of all information related to patients, physicians, colleagues, and visitors at Trinity Health.

• Takes responsibility for performing job duties in the safest manner possible, ensuring personal safety and that of coworkers.

⛳️ Requirements

• High school diploma is required.

• Minimum of one year of post-high school medical education.

• At least one (1) year of clinical oncology healthcare or hospital-based experience.

• Proven interpersonal skills to effectively interact and communicate with patients and medical staff.

• Strong understanding of medical terminology.

• Ability to work autonomously and self-directed with a high level of judgment and decision-making.

• Demonstrated organizational skills necessary for developing and managing multiple monitoring systems.

• Proficiency in computer databases and software, including Excel and Word.

• Prior experience with Epic is preferred.

• Proven ability to effectively solve problems with internal and external customers and systems.

• Must be comfortable working in a collaborative environment.
