This is a fully remote position, open to applicants in New Jersey.

📋 Description

• Conduct timely evaluations of SOC, ROC, and recertification documentation utilizing the CRC assessment tool.

• Offer clinical insights regarding OASIS precision and care plans.

• Assign codes to charts based on primary and secondary reasons for home healthcare.

• Review and expedite 485 documentation within required timeframes.

• Utilize home care information systems along with decision support software.

• Manage resource utilization for Medicare clients and suggest resources that align with patient needs.

• Provide guidance and training to clinical and intake personnel on OASIS and documentation gaps.

• Alert managers regarding staff non-compliance and identify trends necessitating staff education.

• Create and execute educational sessions tailored for individuals and teams.

• Engage in the agency’s QAPI program and participate in performance improvement initiatives.

• Cross-train and accept assignments to support the agency while contributing to its business growth.

• Attend ongoing education related to OASIS reviews and ICD-10 coding.

⛳️ Requirements

• Valid NJ RN or PT License.

• Minimum of three years of experience in Certified Home Care.

• Comprehensive understanding of home care regulations and Conditions of Participation (COPs).

• OASIS and Coding certification is preferred.
