This is a fully remote position, open to applicants in California.
• Oversee the design and implementation of program initiatives, guaranteeing timely completion, strategic alignment, organizational objectives, and measurable results.
• Foster and sustain relationships with internal teams, leadership, and external stakeholders.
• Assist in the rollout of digital health solutions, including remote patient monitoring and patient-facing technologies like patient portals.
• Utilize program and project management methodologies encompassing governance, prioritization, resource allocation, risk management, and quality oversight.
• Employ data and analytics to inform decisions, enhance continuous improvement, and shape program or product roadmaps.
• Act as the primary liaison for program communications, providing updates, presentations, and training sessions.
• Enhance the adoption, execution, and refinement of tools, technologies, and processes.
• Advocate for a health center-focused culture that supports the transformation of health care organizations.
• Stay informed about industry trends, regulatory standards, adult learning methodologies, OCHIN services, and pertinent program insights.
• Interact with health care organizations regarding statuses, timelines, data needs, and expected deliverables.
• Assist health care organizations in overcoming challenges and formulating, executing, and maintaining action plans to enhance rural health care.
• Bachelor's degree in healthcare, program/project management, or a related discipline is required; a master's degree is preferred.
• A minimum of 3 years of experience supporting health care teams in a similar or relevant capacity is strongly preferred; 4–5 years is a plus.
• Experience with patient portals, telehealth, e-consults, and/or remote patient monitoring technology is required.
• Familiarity with Epic functionality is preferred.
• Proficient knowledge of healthcare data, reporting, and analysis is required.
• Understanding of advanced primary care principles, chronic disease management, and clinical quality metrics such as UDS, HEDIS, NCQA, or NQF metrics, as well as FQHC/safety net clinics is preferred.
• Experience in supporting clinicians, care teams, and clinic leaders in implementing practice changes within clinical environments.
• Knowledge of motivational interviewing, human-centered design, Model for Improvement, Lean, PCMH, or Building Blocks of High Performing Primary Care frameworks.
• Strong grasp of the project lifecycle and the capability to independently foresee and resolve issues.
• Proven ability to drive implementation of multidisciplinary, complex, multi-stakeholder healthcare projects and manage numerous projects concurrently.
• Excellent analytical, problem-solving, written, verbal, facilitation, coaching, and consulting abilities.
• Significant experience working in a remote role, including familiarity with OneNote, SharePoint, Teams, Zoom, and Smartsheet; JIRA experience is beneficial.
• Willingness to travel to urban and rural areas up to 25–50%, including nationwide travel for Go-Live support or training.
• Capability to work independently and effectively from a home office with high-speed internet in a distraction-free environment.
• Required proficiency in reading, speaking, writing, and understanding English.
