
PRN Nurse Practitioner, Home Visits/Telehealth

This is a fully remote position, open to applicants in Mississippi.
β’ Perform thorough in-home health risk evaluations to identify both active and chronic medical conditions, as well as assess all physical, mental, and social needs present during the visit.
β’ Gather patient history, conduct examinations, establish diagnoses, and provide written documentation of patient visits in accordance with NCQA standards.
β’ Measure patient vital signs as needed, initiate case management referrals, and communicate with primary care providers when necessary.
β’ Liaise with patients, caregivers, agency nurses, other healthcare providers, and vendors as required to ensure accurate diagnoses.
β’ Carry out all clinical responsibilities while adhering to OSHA Universal Precautions.
β’ Uphold patient confidentiality at all times.
β’ Attend mandatory meetings and training sessions, and engage in committees as requested.
β’ Participate in ongoing professional development activities and maintain professional licenses and affiliations.
β’ Must possess an active/unrestricted nurse practitioner license to practice in the relevant states.
β’ Board certification from one of the following organizations is required: American Nurses Credentialing Center (ANCC), American Association of Nurse Practitioners (AANP), or National Commission on Certification of Physician Assistants (NCCPA).
β’ Must have an active BLS Certification.
β’ Current enrollment in Medicare/Medicaid is necessary.
β’ A valid driverβs license and a clean driving record are required.
β’ Exceptional electronic health record (EHR) skills are essential.
β’ Participation in an Accountable Care Organization.
