PRN Nurse Practitioner, Home Visits/Telehealth

This is a fully remote position, open to applicants in Mississippi.

📋 Description

• Perform thorough in-home health risk evaluations to identify both active and chronic medical conditions, as well as assess all physical, mental, and social needs present during the visit.

• Gather patient history, conduct examinations, establish diagnoses, and provide written documentation of patient visits in accordance with NCQA standards.

• Measure patient vital signs as needed, initiate case management referrals, and communicate with primary care providers when necessary.

• Liaise with patients, caregivers, agency nurses, other healthcare providers, and vendors as required to ensure accurate diagnoses.

• Carry out all clinical responsibilities while adhering to OSHA Universal Precautions.

• Uphold patient confidentiality at all times.

• Attend mandatory meetings and training sessions, and engage in committees as requested.

• Participate in ongoing professional development activities and maintain professional licenses and affiliations.

⛳️ Requirements

• Must possess an active/unrestricted nurse practitioner license to practice in the relevant states.

• Board certification from one of the following organizations is required: American Nurses Credentialing Center (ANCC), American Association of Nurse Practitioners (AANP), or National Commission on Certification of Physician Assistants (NCCPA).

• Must have an active BLS Certification.

• Current enrollment in Medicare/Medicaid is necessary.

• A valid driver’s license and a clean driving record are required.

• Exceptional electronic health record (EHR) skills are essential.
