PRN MTM Pharmacist

This is a fully remote position, open to applicants in United States.

📋 Description

• Conducts thorough telephonic medication reviews for patients.

• Updates patient profiles using the information collected during the review process.

• Identifies and addresses Drug Therapy Problems to optimize patients’ medication regimens and reduce the risk of adverse drug reactions.

• Collaborates with a multidisciplinary team to enhance and develop our MedPro software.

• Achieves a minimum of two comprehensive medication reviews (CMRs) per hour.

• Supports the Clinical team in achieving proficiency and quality objectives.

• Utilizes client formulary data and member-specific benefits to inform suitable medication recommendations.

• Stays informed about emerging trends to educate the clinical team, physicians, and other prescribers.

⛳️ Requirements

• Doctor of Pharmacy degree or an equivalent qualification.

• Previous experience in Medication Therapy Management (MTM) is preferred but not mandatory.

• Excellent oral and written communication abilities.

• Exceptional telephone skills and professional etiquette.

• Strong clinical and analytical capabilities.

• Ability to work autonomously and demonstrate initiative.

• Proficient in Google Docs and Microsoft Excel.

• Keen attention to detail and the ability to adhere to established procedures.
