This is a fully remote position, open to applicants in North Carolina.
• Enhance patient outcomes through telephonic interventions.
• Concentrate on the tactical execution of medication management services, which includes polypharmacy, adherence, and optimization of pharmacotherapy.
• Provide counseling to patients through telephonic outreach regarding medication adherence, addressing medication-related issues such as barriers to adherence, polypharmacy, or the use of high-risk medications, while making suitable evidence-based recommendations to patients and Practice Care Teams.
• Conduct focused clinical chart reviews as necessary to inform patient interventions.
• Secure and document patient commitment to medication adherence and tackle barriers in real-time.
• Refer complex clinical cases to prescribers or interdisciplinary teams as needed.
• Achieve and maintain performance goals (e.g., STARs, HEDIS).
• Collaborate with Physicians and clinical and administrative staff to enhance the Medication Management workflow and patient experience.
• Proactively identify gaps in medication management through patient interactions and convert patient insights into specific recommendations that drive enhancements in practice operations and clinical workflows.
• Engage in the design, implementation, and assessment of pilot programs or process modifications (“test and learn”) within the outreach model.
• Act as a clinical subject matter expert, utilizing clinical pharmacy expertise to support organizational objectives and adapt to the evolving needs of healthcare.
• Create and maintain standardized resources, educational tools, and clinical protocols to improve medication outcomes and performance on quality measures (e.g., STARS, HEDIS) for practices, clinicians, and patients.
• Doctor of Pharmacy (PharmD).
• Active, unrestricted pharmacist license in the state of practice; valid multi-state licensure in at least two of the following states: AL, KY, LA, MS, NC, KS, PA, UT, VA, WV, MI, DE.
• Willingness to acquire and maintain additional licenses within 6 months of hire.
• A minimum of 3 years of experience in value-based care, ambulatory care, or community pharmacy settings.
• Comprehensive knowledge of pharmacotherapy and pharmacy practice.
• Excellent verbal and written communication skills; ability to translate complex clinical data into clear, understandable language for patients and care teams.
• Proficient in telephony platforms and various Electronic Health Record (EHR) systems.
• Capacity to work in a fast-paced environment and adapt swiftly to change.
• Availability to work between 8 AM - 7 PM EST as required by business needs.
