πŸ“‹ Description

β€’ The Clinical Pharmacist is essential in enhancing the quality, safety, and efficacy of pharmacotherapy for Healthspring Medicare members.

β€’ Work collaboratively with a high-performing, multidisciplinary team of professionals across various functions.

β€’ Perform comprehensive medication reviews (CMRs) for at-risk populations.

β€’ Evaluate drug interactions, identify therapeutic duplications, and assess renal and hepatic dosing adjustments.

β€’ Address evidence-based care gaps and support STAR-related pharmacy initiatives.

β€’ Identify obstacles to medication affordability and patient adherence.


⛳️ Requirements

β€’ An active and unrestricted Pharmacist license in the state of residence is required.

β€’ A Doctor of Pharmacy (PharmD) degree is a necessity.

β€’ Possess strong clinical medication knowledge with the capacity to implement evidence-based guidelines in practice.

β€’ A minimum of 3 years of experience in Managed Care, Medicare, and/or Medicaid settings is preferred.

β€’ Experience with Medication Therapy Management (MTM) programs is highly preferred.

β€’ Familiarity with MTM platforms (e.g., Adhere Health’s Optimize or similar applications) is preferred.

β€’ Exceptional verbal and written communication skills, with the ability to effectively engage diverse audiences.

β€’ Bilingual proficiency in English and Spanish is required.

β€’ Advanced proficiency in Microsoft Office applications, including Word, Excel, PowerPoint, and Outlook.

β€’ Strong computer literacy with the ability to navigate multiple systems and applications simultaneously.

β€’ Demonstrate strong learning agility and the ability to implement new concepts and tools.

β€’ Possess strong analytical thinking, problem-solving, and clinical decision-making abilities.

β€’ Capability to develop innovative solutions to complex healthcare challenges.
