This is a fully remote position, open to applicants in California.
• Conduct telephonic, video, and SMS outreach, engagement, and education for vulnerable patients enrolled in the Medicaid program in California.
• Collaborate with a multidisciplinary team to deliver care management services that complement primary care provider practices; team members include Community Health Workers, Care Coordinators, and Behavioral Health Therapists.
• Assist patients and providers during transitions of care.
• Deliver clinical services that facilitate the daily operations and ensure safe practices within the program.
• Engage in quality improvement activities to ensure quality, cost-effectiveness, and return on investment (ROI).
• Leverage analytics dashboards to proactively reach out to patients experiencing medication and/or equipment rejections, admissions/readmissions, or who could benefit from medication adherence counseling.
• Mentor and lead pharmacists and/or pharmacy technicians.
• PharmD with an advanced practice license in good standing in California.
• Successful completion of an accredited PGY1 residency training in a relevant discipline.
• Over 5 years of experience in pharmacy practice, including direct patient care interventions in an ambulatory setting for vulnerable populations.
• Proven experience in multidisciplinary care management.
• Positive references from previous colleagues.
• Preferred: Completion of PGY2 residency training in a related discipline (ambulatory care, geriatrics, pharmacotherapy, internal medicine).
• Strong analytical skills with the ability to navigate dashboards and registry data for proactive outreach campaigns, including addressing HEDIS gaps in care preferred.
• Preferred: Experience in medication management through collaborative practice agreements with primary care providers.
• Preferred: Leadership experience in supporting other pharmacists, pharmacy technicians, and/or other care team members.
