This is a fully remote position, open to applicants in Massachusetts.
• Deliver exceptional preventative, acute, and chronic care in a fully remote, virtual environment.
• Enhance the quality of remote care by offering compassionate and impactful patient interactions.
• Oversee a dedicated group of patients as their primary care physician.
• Refer patients to specialists or acute care services when clinically necessary.
• Participate in population health initiatives for risk-attributed patients (AWVs, care gaps, transitions of care, risk capture).
• Assist patients immediately following discharge to clarify treatment plans and minimize hospital readmissions.
• Engage in ongoing self-assessment to ensure compliance with clinical guidelines and best practices.
• Board-Certified in Family Medicine or Internal Medicine.
• Minimum of 3 years of post-residency professional experience.
• At least 2 years of experience in a traditional primary care setting.
• Experience with virtual consultations is mandatory.
• Must hold an active medical license in Massachusetts; IMLC is preferred.
• Prior experience with EPIC EMR is required.
• Clean background and a history free from medical malpractice.
• Must be currently enrolled or eligible for enrollment as a Medicare provider.
• Willingness to commit to 40 hours per week, with 36 hours dedicated to clinical duties.
• Must be available to work on Mondays and Fridays with flexible starting hours (e.g., 7 am-12:30 pm EST).
• Bilingual in Spanish is a plus.
• Strong enthusiasm for building long-term relationships with patients and managing chronic care conditions.
• Comfortable managing acute conditions in a virtual format.
• Extensive experience managing more than 3 cases per hour.
• Eagerness to learn and implement innovative technology to enhance efficiency and clinical results.
