Primary Care Physician – FM

This is a fully remote position, open to applicants in Connecticut, +3 more states.

📋 Description

• Deliver exceptional preventative, acute, and chronic care within a fully-remote, virtual environment.

• Enhance remote healthcare by offering compassionate and significant patient interactions.

• Oversee a dedicated panel of patients as their primary care provider.

• Refer patients to specialists or acute care services when clinically necessary.

• Engage in population health initiatives for risk-attributed patients (AWVs, care gaps, transitions of care, risk capture).

• Assist patients immediately following discharge to improve comprehension of treatment plans and decrease hospital readmissions.

• Practice ongoing self-evaluation to assure compliance with clinical guidelines and best practices.

⛳️ Requirements

• A minimum of 3 years of experience post-residency.

• At least 2 years of experience in an in-person primary care environment.

• Experience with virtual consultations is mandatory.

• Active state license in NJ is required; licenses in CT, PA, NY & IMLC are preferred.

• Previous experience with EPIC EMR is essential.

• A clean background and a history free of medical malpractice is required.

• Must be prepared to commit to 40 hours per week, with 36 clinical hours.

• Availability to work on Mondays and Fridays with flexible starting hours (e.g., 7 am-12:30 pm ET).

• Proficiency in Spanish is a plus.

• A strong desire to build long-term relationships with patients and manage chronic care conditions.

• Comfort in managing acute conditions within a virtual framework.

• Extensive experience handling more than 3 cases per hour.
