Pre-Service RN Care Coordinator

This is a fully remote position, open to applicants in United States.

📋 Description

• Execute clinical and operational tasks associated with the transition of care, care coordination, and case management for post-acute care services, which encompass home health, durable medical equipment, and related care management offerings.

• Assess and coordinate services or equipment prescribed by the physician to ensure timely and appropriate member care.

• Work collaboratively with providers, members, and internal teams to uphold continuity of care and address service coordination requirements.

• Accurately document activities and adhere to established clinical, operational, and compliance protocols.

⛳️ Requirements

• Possession of an active, unrestricted Registered Nurse license in the state of residence.

• At least 2 years of clinical RN experience.

• Availability to work Monday through Friday, 9:00 a.m.–6:00 p.m. Central Time, with flexibility to work any shift between 7:00 a.m. and 7:00 p.m. Central Time based on business requirements.

• Willingness to participate in weekend, on-call, and holiday rotations as necessary.

• Proficient in using a computer, telephone, and other standard office equipment throughout the workday.

• Capability to maintain a consistent and reliable work schedule.

• Bachelor of Science in Nursing is preferred.

• Multistate RN license is preferred.

• 2 or more years of utilization management experience is preferred.

• Experience in home health, skilled nursing facilities, rehabilitation, post-acute care, or care coordination is preferred.

• Familiarity with Medicare and/or Medicaid managed care programs is preferred.
