
Pre-Service RN Care Coordinator

This is a fully remote position, open to applicants in United States.
β’ Execute clinical and operational tasks associated with the transition of care, care coordination, and case management for post-acute care services, which encompass home health, durable medical equipment, and related care management offerings.
β’ Assess and coordinate services or equipment prescribed by the physician to ensure timely and appropriate member care.
β’ Work collaboratively with providers, members, and internal teams to uphold continuity of care and address service coordination requirements.
β’ Accurately document activities and adhere to established clinical, operational, and compliance protocols.
β’ Possession of an active, unrestricted Registered Nurse license in the state of residence.
β’ At least 2 years of clinical RN experience.
β’ Availability to work Monday through Friday, 9:00 a.m.β6:00 p.m. Central Time, with flexibility to work any shift between 7:00 a.m. and 7:00 p.m. Central Time based on business requirements.
β’ Willingness to participate in weekend, on-call, and holiday rotations as necessary.
β’ Proficient in using a computer, telephone, and other standard office equipment throughout the workday.
β’ Capability to maintain a consistent and reliable work schedule.
β’ Bachelor of Science in Nursing is preferred.
β’ Multistate RN license is preferred.
β’ 2 or more years of utilization management experience is preferred.
β’ Experience in home health, skilled nursing facilities, rehabilitation, post-acute care, or care coordination is preferred.
β’ Familiarity with Medicare and/or Medicaid managed care programs is preferred.
