This is a fully remote position, open to applicants in California.
• Act as the main liaison between Vytalize Health and designated provider practices.
• Spearhead the implementation and rollout of value-based care models, which include population health management, practice transformation, care coordination, and risk-based contracting.
• Work in collaboration with providers and practice personnel to enhance clinical and administrative workflows using evidence-based guidelines, care pathways, and quality improvement frameworks.
• Implement and optimize the utilization of technology solutions in practices, such as EHRs, patient portals, and care management tools.
• Ensure adherence to all pertinent regulations and accreditations related to value-based care.
• Develop and execute population health management programs aimed at addressing the needs of high-risk patient populations and enhancing patient engagement.
• Collaborate with practices to identify and implement strategies to achieve quality performance objectives.
• Facilitate care coordination among healthcare providers, patients, and families to ensure a seamless care delivery process.
• Evaluate and manage risks associated with value-based care contracts.
• Create and implement performance measurement systems to assess the effectiveness of VBC interventions and monitor accountability within practices.
• Establish robust, trust-based relationships with practice administrators, providers, and staff.
• Bachelor’s degree in Healthcare Administration, Public Health, Nursing, or a relevant field.
• A minimum of 3 years of experience in practice transformation, care coordination, population health, or a similar healthcare operations role.
• Direct experience working with physician practices or provider organizations within a value-based care framework.
• Strong knowledge of value-based care models and operations within primary care practices.
• Ability to manage multiple practice relationships simultaneously while delivering measurable outcomes.
• Effective communicator with exceptional presentation, follow-up, and stakeholder management skills.
• Proficient in working with data and performance metrics to inform practice discussions and interventions.
• Clinical background or direct experience in a primary care or ambulatory care environment (preferred).
• Familiarity with EMR systems, HEDIS measures, and population health management platforms (preferred).
• Experience with ACO, MSSP, or private payer value-based care program requirements (preferred).
