This is a fully remote position, open to applicants in United States.
• Oversees the development of workflows and tools, process enhancements, implementation, and performance tracking for essential Population Health projects.
• Acts as the organization-wide program manager for specified initiatives.
• Collaborates closely with market-level population health teams and leaders to facilitate the achievement of nursing and case management results.
• Creates and sustains enterprise-wide processes and programs in partnership with Directors and Vice Presidents of Population Health.
• Functions as a business owner in collaboration with IT and Analytics teams.
• Guarantees that tools and reporting solutions are tailored to fulfill operational and clinical business requirements.
• Principal responsibilities include leading user acceptance testing and data validation efforts, as well as monitoring workflow effectiveness.
• Works with cross-functional program leaders to enhance implementation processes and reporting over time.
• Provides indirect leadership through the creation and execution of training and train-the-trainer sessions.
• Current and active clinical licensure in good standing.
• Bachelor’s degree is mandatory.
• Master’s degree in technology or clinical informatics is preferred.
• At least 5 years of experience in a clinical environment, including a minimum of 3 years in population health within a value-based care context.
• A minimum of 2 years in Clinical Informatics and/or Analytics is required, with a preference for candidates with experience in productivity and KPI tracking.
• Proven track record in clinical program design, implementation, and leadership within a multi-location, preferably national framework.
• Strong project management, analytical, and problem-solving abilities, employing a data-driven approach to optimize workflows, processes, and implementations.
• Demonstrated expertise in data validation, reporting, analysis, data-driven decision-making, and issue identification and tracking.
• Experience as a business owner for technology solutions, encompassing requirement development, solution testing, and data validation.
• Detail-oriented with excellent problem-solving and organizational skills.
• Highly effective in both written and verbal communication.
• Professional demeanor, capable of effectively responding to rapidly evolving internal and external circumstances at ConcertoCare.
• Proven ability to manage multiple concurrent tasks with minimal supervision and meet deadlines.
• Capable of thriving in a fast-paced, dynamic environment and collaborating effectively within a team.
• Proficient in computer applications, including the Microsoft Office Suite (Outlook, Excel, PowerPoint, Word).
• Expert-level proficiency in clinical documentation and reporting systems, including EMRs such as eCW and reporting tools like Power BI.
